What This Test Measures

Serum progesterone measured by liquid chromatography-tandem mass spectrometry (LC-MS/MS). More accurate than immunoassay for women on bioidentical hormone therapy (BHRT), as standard immunoassay may cross-react with synthetic progestins and give inaccurate results. Preferred method for monitoring progesterone in BHRT patients.

Preparation

No fasting required
Timing matters: for women not on BHRT, draw on day 21 of cycle (mid-luteal). For BHRT users, check with your provider — timing depends on delivery method and individual protocol.
💬 Patient Community Notes

Recommended for oral progesterone users (Prometrium/micronized progesterone): standard immunoassay can overestimate levels due to cross-reactivity with progesterone metabolites. LC-MS/MS is more accurate for monitoring oral progesterone.
