Estradiol (E2)

CPT: 82670
Hormones

What This Test Measures

Most potent estrogen; primary sex hormone test for women and estrogen monitoring in men on TRT.

Preparation

No fasting required
Draw at a consistent point in your cycle if cycling (typically Day 3 for baseline, or Day 21 for luteal phase progesterone). If on BHRT injections, draw at trough (just before next injection). Results vary significantly by time in cycle.
๐Ÿ’ฌ Patient Community Notes

TIMING: For cycling women, estradiol varies enormously by cycle day โ€” specify your cycle day on the lab order. Day 3 for baseline (follicular), day 12โ€“14 for peak. For BHRT: draw at trough (just before your next injection/dose) for stable baseline. The BHRT community notes that levels vary widely by delivery method โ€” ranges 'normal' by standard labs (typically <200 pg/mL) may be subtherapeutic for women who feel best at higher levels. Symptom correlation matters as much as numbers. Topical estradiol users: test at trough, not right after application. These notes reflect common practices in BHRT/functional medicine communities.

Reflects common practices in thyroid, BHRT, and functional medicine communities. This may differ from standard clinical guidelines. Not medical advice โ€” always discuss with your provider.

General guidelines only. Requirements vary by lab and individual. Confirm with your provider before adjusting any medications.
